TERMS OF CANCELLATION: (as required by your Membership Agreement)
- CANCELLATION FORM: If you are sure that you would like to cancel your membership, just complete and submit the Cancellation Form below.
- WRITTEN CANCELLATION NOTICE: This will serve as your Written Cancellation Notice as required by your membership agreement. Note that if you have a scheduled Auto-Payment within the next 15-day period, that payment will be processed as scheduled. All payments are non-refundable. You may continue to use your active membership for the remainder of the time-frame that has been paid.
- CANCELLATION FEE: Per your Membership Agreement, cancellation of your “Commitment Pricing Membership” will result in a flat $65 cancellation fee that will be automatically charged to your account when you submit this Cancellation Form.
